Safety has traditionally been defined as a condition where the number of adverse outcomes was as low as possible (Safety-I). From a Safety-I perspective, the purpose of safety management is to make sure that the number of accidents and incidents is kept as low as possible, or as low as is reasonably practicable. This means that safety management must start from the manifestations of the absence of safety and that - paradoxically - safety is measured by counting the number of cases where it fails rather than by the number of cases where it succeeds. This unavoidably leads to a reactive approach based on responding to what goes wrong or what is identified as a risk - as something that could go wrong. Focusing on what goes right, rather than on what goes wrong, changes the definition of safety from ‘avoiding that something goes wrong’ to ‘ensuring that everything goes right’. More precisely, Safety-II is the ability to succeed under varying conditions, so that the number of intended and acceptable outcomes is as high as possible. From a Safety-II perspective, the purpose of safety management is to ensure that as much as possible goes right, in the sense that everyday work achieves its objectives. This means that safety is managed by what it achieves (successes, things that go right), and that likewise it is measured by counting the number of cases where things go right. In order to do this, safety management cannot only be reactive, it must also be proactive. But it must be proactive with regard to how actions succeed, to everyday acceptable performance, rather than with regard to how they can fail, as traditional risk analysis does. This book analyses and explains the principles behind both approaches and uses this to consider the past and future of safety management practices. The analysis makes use of common examples and cases from domains such as aviation, nuclear power production, process management and health care. The final chapters explain the theoretical and practical consequences of the new perspective on the level of day-to-day operations as well as on the level of strategic management (safety culture). Safety-I and Safety-II is written for all professionals responsible for their organisation's safety, from strategic planning on the executive level to day-to-day operations in the field. It presents the detailed and tested arguments for a transformation from protective to productive safety management.

While the Obama administration's Patient Protection and Affordable Care Act (ACA) has expanded health care coverage for millions of Americans, it has fallen short in offering universal health care to all. In Health Care as a Right of Citizenship, Gunnar Almgren argues that the ACA's primary significance is not in its expansion of health care entitlements but in its affirming by an act of Congress the idea that comprehensive health care must be available to all as a right of citizenship. Residents of Haiti-one of the poorest and most unstable countries in the world-face a grim reality of starvation, violence, lack of economic opportunity, and minimal health care. For years, aid organizations have sought to alleviate the problems by creating health and family planning clinics, including one modern (and, by local standards, luxurious) center in the heart of Cit Soleil. During its height of service in the 1980s and 1990s, the clinic boasted nineteen staff members, an array of modern contraceptives, an accessible location, and convenient hours-but very few clients. Why did this initiative fail so spectacularly despite surveys finding that residents would like to have fewer children? Why don't poor women heed the message of family planning, when smaller families seem to be in their best interest? In Reproducing Inequities, M. Catherine Maternowska argues that we too easily overlook the political dynamics that shape choices about family planning. Through a detailed study of the attempt to provide modern contraception in the community of Cit Soleil, Maternowska demonstrates the complex interplay between local and global politics that so often thwarts well-intended policy initiatives. Medical anthropologists, she argues, have an important role to play in developing new action plans for better policy implementation. Rethinking Urban Transitions provides critical insight for societal and policy debates about the potential and limits of low carbon urbanism. It draws on over a decade of international research, undertaken by scholars across multiple disciplines concerned with analysing and shaping urban sustainability transitions. It seeks to open up the possibility of a new generation of urban low carbon transition research, which foregrounds the importance of political, geographical and developmental context in shaping the possibilities for a low carbon urban future. The book’s contributions propose an interpretation of urban low carbon transitions as primarily social, political and developmental processes. Rather than being primarily technical efforts aimed at measuring and mitigating greenhouse gases, the low carbon transition requires a shift in the mode and politics of urban development. The book argues that moving towards this model requires rethinking what it means to design, practise and mobilize low carbon in the city, while also acknowledging the presence of multiple and contested developmental pathways. Key to this shift is thinking about transitions, not solely as technical, infrastructural or systemic shifts, but also as a way of thinking about collective futures, societal development and governing modes – a recognition of the political and contested nature of low carbon urbanism. The various contributions provide novel conceptual frameworks as well as empirically rich cases through which we can begin to interrogate the relevance of socio-economic, political and developmental dimensions in the making or unmaking of low carbon in the city. The book draws on a diverse range of examples (including ‘world cities’ and ‘ordinary cities’) from North America, South America, Europe, Australia, Africa, India and China, to provide evidence that expectations, aspirations and plans to undertake purposive socio-technical transitions are both emerging and encountering resistance in different urban contexts. Rethinking Urban Transitions is an essential text for courses concerned with cities, climate change and environmental issues in sociology, politics, urban studies, planning, environmental studies, geography and the built environment.

This book explores the significance of human behaviour to understanding the causes and impacts of changing climates and to assessing varied ways of responding to such changes. So far the discipline that has represented and modelled such human behaviour is economics. By contrast Climate Change and Society tries to place the ?social? at the heart of both the analysis of climates and of the assessment of alternative futures. It demonstrates the importance of social practices organised into systems. In the fateful twentieth century various interlocking high carbon systems were established. This sedimented high carbon social practices, engendering huge population growth, increasing greenhouse gas emissions and the potentially declining availability of oil that made this world go round. Especially important in stabilising this pattern was the ?carbon military-industrial complex? around the world. The book goes on to examine how in this new century it is systems that have to change, to move from growing high carbon systems to those that are low carbon. Many suggestions are made as to how to innovate such low carbon systems. It is shown that such a transition has to happen fast so as to create positive feedbacks of each low carbon system upon each other. Various scenarios are elaborated of differing futures for the middle of this century, futures that all contain significant costs for the scale, extent and richness of social life. Climate Change and Society thus attempts to replace economics with sociology as the dominant discipline in climate change analysis. Sociology has spent much time examining the nature of modern societies, of modernity, but mostly failed to analyse the carbon resource base of such societies. This book seeks to remedy that failing. It should appeal to teachers and students in sociology, economics, environmental studies, geography, planning, politics and science studies, as well as to the public concerned with the long term future of carbon and society.
